National Healthcare Decisions Day (NHDD), occurring April 16 each year, exists to inspire, educate and empower the public and providers about the importance of advance care planning.

NHDD, now run by The Conversation Project (TCP), is an initiative to encourage patients to express their wishes regarding healthcare, and for providers and facilities to respect those wishes, whatever they may be.

Advance Care Planning tools and resources are available on TCP's website, including TCP's Your Conversation Starter Guide and What Matters to Me Workbook.

Relevant resources from other JAHF grantees and partners related to advance care planning include:

  • Prepare for your Care's resources for families navigating medical decision making
  • Fast-Forward documentary and related courses and a digital toolkit of resources and master checklist from Next Avenue
  • The Better Care Playbook resource post on NYU Langone Health's study, Supporting Acute Advance Care Planning with Precise, Timely Mortality Risk Predictions
  • Health Affairs paper, Advance Care Planning For Medicare Beneficiaries Increased Substantially, But Prevalence Remained Low
  • The New York Times article, The Power (and Joy) of Being Prepared
  • The New York Times article, Filing Suit for ‘Wrongful Life’ and National POLST's press release in response
  • The Center to Advance Palliative Care and VitalTalk's Conversation scripts and video examples of how to talk to patients and families about advance care planning
