NCQA Blog: NCQA Tests Person-Centered Outcome Measures With D-SNP Plans
The National Committee for Quality Assurance (NCQA) published a blog on recent testing of Person-Centered Outcome (PCO) measures in Dual-Eligible Special Needs Plans (D-SNPs).
The blog highlights insights from the testing, including how D-SNPs and Chronic Condition Special Needs Plans (C-SNPs) can integrate goal-driven, person-centered outcomes into existing workflows, supporting care that reflects what matters most to people with complex health needs.
As PCO measures move through the HEDIS® approval process for Measurement Year 2027, this work signals a shift toward measuring progress beyond clinical indicators and advancing more person-centered, age-friendly care.
Over the past decade, NCQA has developed these measures with support from JAHF, The SCAN Foundation, and the Gordon and Betty Moore Foundation.
