NDCC Insights Webinar: Home and Community-Based Services for Comprehensive Dementia Care
The National Dementia Care Collaborative (NDCC) is hosting a webinar on practical ways to strengthen connections between home and community-based service (HCBS) providers and clinical care providers.
Speakers from Aging & In-Home Services (AIHS) of Northeast Indiana will share how their organization turns person-centered care plans into tailored support for people living with dementia and their family caregivers. The team will share results from its work with GUIDE beneficiaries and offer tools to help increase HCBS use—whether you participate in GUIDE or support the model as a dementia care provider, primary care clinician or respite partner.
