The National Dementia Care Collaborative (NDCC) is holding session eight of its webinar series on August 21, "Getting to Yes for Launch: Overcoming Challenges to Implement Comprehensive Dementia Care Assessments & Care Planning."

This webinar is part of a series focused on discussions to assist new and established sites implementing the Guiding an Improved Dementia Experience (GUIDE) Model. Previous webinars are recorded, and the slides are available.

Presenters from the University of Oklahoma and Johns Hopkins University, both GUIDE Established Track Programs, will share their experience on completing a comprehensive assessment of the person living with dementia, a comprehensive caregiver assessment, and the development and use of a person-centered care plan.
