New National Dementia Care Collaborative and Summit
The Education Development Center (EDC) has launched the National Dementia Care Collaborative (NDCC) with a new website at ndcc.edc.org, and will be hosting its Autumn Summit November 28 on, "The CMS GUIDE Model: Choices for Improving Evidence-Based Dementia Care."
NDCC aims to improve access to evidence-based comprehensive dementia care. The Autumn Summit is a virtual event, free and open to all. During the Summit, presenters will discuss the Guiding an Improved Dementia Experience (GUIDE) Model from the Centers for Medicare & Medicaid Services Innovation Center. The GUIDE Model seeks to support people living with dementia and their family caregivers and care partners, to promote comprehensive, high-quality dementia care, and to address health disparities in dementia.
Summit participants will:
- Learn how available evidence-based, comprehensive dementia care models can be implemented to help meet GUIDE Model requirements.
- Learn about the GUIDE Model application process with insights from the Center for Medicare and Medicaid Innovation (CMMI).
- Consider which evidence-based models align with different settings and needs.
- Hear directly from evidence-based model developers and other national leaders, including opportunities to ask questions in breakout groups.
