NRHA Webinar Connected Care for Older Adults

The National Rural Health Association (NRHA) held a Connected Care for Older Adults webinar on October 16 titled, "Community Health Workers & Age-Friendly Care: An Innovative Pilot in Rural Oregon."

Connected Care for Older Adults is an innovative model that uses Community Health Workers (CHWs) in partnership with primary care providers (PCPs) to improve care for frail older adults in rural areas. Through Connected Care, CHWs trained in Age-Friendly Health Systems conduct a series of home visits with patients and families and implement Connected Care protocols based on the 4Ms Framework.

This webinar provided quantitative data and anecdotal findings about the power of this model to improve primary care services and outcomes for at-risk older adult patients in rural areas. The webinar was presented in partnership with Oregon Health & Science University and the National Rural Age-Friendly Initiative.

  • Lindsay Miller, MBA, project director, Connected Care for Older Adults, a project of the Columbia Gorge Health Council
