The Community Professionals Toolkit includes a collection of tools on identifying health priorities geared toward senior centers, area agencies on aging, public health departments, libraries and others.
The newest resource is related to guidance on educating others about My Health Priorities. This consumer-facing presentation is designed for professionals to present to a group of older adults and their caregivers about My Health Priorities.
My Health Priorities is a web questionnaire that guides older adults and their care partners to understand what matters most to them and what care preferences they have.
