Patient Priorities Care Resources and Training: Community Professional Tools, Fall Telementoring Series and Self-Directed Training
Patient Priorities Care has announced its fall 2025 Telementoring Series and updated training resources for health care professionals, including its Self-directed Training and Community Professional Tools.
The fall 2025 Telementoring training is a virtual workshop series that provides health care professionals the space to practice Patient Priorities Care through discussion and case-based coaching. The cohort meets over six, one-hour sessions:
- October 7, 21
- November 4, 18
- December 2, 16
- January 6, 2026 (optional clinician-educator session)
These interactive sessions expand on the core trainings with real case discussions from participants and are open to health professionals and educators of all disciplines.
Patient Priorities Care also offers Self-directed Training through practical modules, conversation guides and case examples that support integrating this approach into everyday practice.
Continuing education (CE) credit opportunities are available through the American College of Physicians and the American Association of Nurse Practitioners. Specialized trainings are also available for the U.S. Department of Veterans Affairs and the Program of All-Inclusive Care for the Elderly (PACE).
In addition, Community Professional Tools provide guidance for professionals working in community-based organizations such as senior centers, area agencies on aging, public health departments, and others, to implement Patient Priorities Care.
