Patient Priorities Care: Updated Website and Map of Sites
Patient Priorities Care, an approach to aligning care among clinicians with what matters most to their patients, has updated its website for patients, clinicians and health systems, including its About section, map of sites, health professional toolkit and other resources.
- The map of sites includes outpatient clinics, PACE organizations, Veterans Affairs facilities, emergency departments, trainee programs and more. Let the team know if you’ve implemented Patient Priorities Care and your site is not listed.
- The new narrative chronicles the history of Patient Priorities Care on the About page, and a Stay Informed section that showcases publications, news stories and archived newsletters. Patient Priorities Care is an evidence-based approach to helping older adults identify their unique health priorities and align care with those priorities, a key component of age-friendly care and the Age-Friendly Health Systems 4Ms Framework.
Further updates include:
- Redesigned health professional toolkit (previously referred to as the implementation toolkit). This suite of tools includes Self-Directed Health Priorities Identification Resources and Guides, including:
- MyHealthPriorities.org: Self-Directed Priorities Identification Website
- Manual for Self-Directed Priorities Identification: guide for all settings
- Mis Prioridades de Salud Guía: guide in Spanish
- Training webpage provides an overview of Patient Priorities Care training modules and live telementoring workshops.
- Patient/Caregiver webpage focuses primarily on health priorities identification and sharing those priorities with their care team and is tailored to the needs of patients and caregivers.
In addition, the team participated in a public access Comcast TV segment - Priorities-Aligned Decision-Making: A Roundtable Discussion - that aired April 25, 2024. The roundtable discussion featured Jessica Esterson, Dr. Jennifer Ouellet, Dr. Barry Wu and Kizzy Hernandez-Bigos who discussed the benefits of identifying health priorities and aligning care and decision-making around those priorities.
