Patient Priorities Care (PPC) has released a new toolkit on guiding PPC implementation within Veterans Affairs (VA) clinical settings to help identify and make treatment decisions based on what matters to older veterans.
Patient Priorities Care aims to align decision-making in all clinical processes with patient priorities. The newly released toolkit focuses on implementing Patient Priorities Care principles in VA sites, and it provides tailored training modules, tools, and resources for every member of a clinical team. Conversation starter packs, practice tip sheets, and other granular resources bring Patient Priorities Care's strategic guidance into implementation. The toolkit's structure is:
