Learn about and influence the Practice Change Networks of the John A. Hartford Foundation Change AGEnts Initiative! The John A. Hartford Foundation Change AGEnts Initiative accelerates sustained practice change that improves the health of older Americans, their families, and communities. Part of this effort, two Practice Change Networks are currently working on several innovative projects related to Dementia Caregiving and Patient Centered Medical Homes. Are you working on research or efforts related to either of these critical issues? Want to be a force for change? Join Network leaders for an update on this exciting work and then participate in small group discussions that will help shape these dynamic Networks’ key activities, evidence-based products, and plans to drive practice and policy change.
