Project BOOST in the News, Helping Milwaukee Area Hospitals Reduce Hospital Readmissions

The Society of Hospital Medicine's Project BOOST (Better Outcomes for Older Adults through Safe Transitions), developed with funding by the Hartford Foundation, was cited in an Aug. 25th Milwaukee Journal Sentinal article by Guy Boulton. BOOST provided the starting point for the Aurora Medical Center's efforts to reduce unnecessary 30-day hosptial readmissions, for which hospitals will be penalized under new Medicare rules beginning Oct. 1.
