Publication: Partnering with PACE - AAAs Leverage Community-based Care Model to Serve Older Adults

AAA PACE

The Aging and Disability Business Institute and the National PACE Association have released a new publication, "Partnering with PACE: AAAs Leverage Community-based Care Model to Serve Older Adults," about how three Area Agencies on Aging (AAAs) have leveraged partnerships with Programs of All-Inclusive Care for the Elderly (PACE) to serve older adults.

The resource presents three case studies that highlight how AAAs can work with PACE in different capacities to provide much-needed community-based care options for older adults. These case studies highlight the myriad opportunities for collaboration between AAAs and PACE and how, when they both work together, they can support older adults with complex care needs in a community-based setting and provide more options for older adults wishing to live in their homes and communities.

The case studies feature CareLink, a AAA serving central Arkansas, Cattaraugus County Department of the Aging, a AAA in rural western New York, and Greater Lynn Senior Services (GLSS), a AAA serving the North Shore of Boston.
