CAPC Report: Serious Illness Strategies - Driving Value in High-Need Populations
The Center to Advance Palliative Care (CAPC) has released a report, Serious Illness Strategies: Driving Value in High-Need Populations, that highlights the most useful strategies, along with practical tips and cautions, gathered from CAPC's Medicare Advantage and Accountable Care Organizations Learning Communities.
The report defines a core set of feasible strategies that make a significant difference in both quality of life and health care spending for people with serious illness:
- proactively identify rising-risk patients
- engage with these individuals and assess their needs
- provide home-based support to avert emergencies
Combining these serious illness strategies produces impressive results for a variety of healthcare organizations, as well as patients and families. These strategies align with the principles of Age-Friendly Health Systems, and many organizations have used this guidance to advance Age-Friendly principles.
