Skilled Nursing News published an article, "Targeting Seniors in the ER Can Bolster Partnerships, Reap Savings" that features the Geriatric Emergency Department Innovations (GEDI WISE) program.
"The ultimate goal of the GEDI WISE program is to facilitate improving patient transitions to skilled nursing facilities (SNFs) or sub-acute rehab, Ula Hwang said." A recent study by Dr. Hwang and colleagues found that emergency departments could play a key role in care transitions for older patients and lead to overall Medicare savings. The GEDI WISE program was found to prevent unnecessary hospital admissions by as much as 33%.
