The Institute for Healthcare Improvement (IHI) has published a new guide, "Age-Friendly Health Systems State-Based Guide: Transforming Health Care for Older Adults Across Your State."

A place-based approach to care delivery is essential for improving the health and quality of life of older adults. This guide provides a practical framework for states and regions to scale the Age-Friendly Health Systems 4Ms Framework—What Matters, Medication, Mentation, and Mobility—through four actionable levers built on a foundation of health equity.

The four key levers help organizations:

• Deepen the practice of the 4Ms and expand age-friendly care to new networks.
  
• Empower champions to lead and sustain the movement.
  
• Adapt proven strategies to fit unique contexts.
