The Multiple Chronic Conditions Resource Center (MCCRC) has released a new podcast episode, via Healthcom Media, with Cheryl Phillips, MD, AGSF, senior program consultant at The John A. Hartford Foundation (JAHF).

Dr. Phillips talks about the Age-Friendly Health Systems initiative 4Ms Framework (What Matters, Medication, Mentation and Mobility) and how these concepts interface with person-centered care planning for persons with and at risk for multiple chronic conditions.

In addition, Dr. Phillips describes the administration’s policy changes with the Agency for Healthcare Research and Quality (AHRQ) and explains that JAHF will continue to promote person-centered care for the nation's aging population.
