The National Home-Based Primary Care Learning Network: Request for Applications
The National Home-Based Primary Care Learning Network is accepting applications for their next wave of practices to join the Learning Network, an 8-month-long experiential quality improvement intensive learning collaborative.
The National Home-Based Primary Care Learning Network, supported and funded by The John A. Hartford Foundation, has led efforts over the past few years to improve care delivery in home-based primary care (HBPC). The Learning Network comprises a national community of HBPC practices that seeks to improve medical practice in the home and expand the evidence base for home-based medical care.
The Learning Network is seeking applications for up to 10 home-based primary care practices from across the country to join the 45 current Learning Network practices across 27 states who have joined this community, and many who have continued in the learning community beyond the 8-month intensive collaborative.
The new deadline for applications is February 17.
