The National Home-Based Primary Care Learning Network: Wave 10 Request for Applications
The National Home-Based Primary Care (HBPC) Learning Network has announced their Wave 10 request for applications, due January 5, 2026.
The HBPC Learning Network is an experiential quality improvement intensive learning collaborative. In this round, the Learning Network will add up to 15 HBPC practices from across the country to join the 102 Learning Network practices in the community. The Learning Network:
- Provides strategies and coaching support to help HBPC practices implement quality improvement principles.
- Supports practices in demonstrating high-value care.
- Helps practices and clinicians connect and build community.
- Provides complimentary access to a quality improvement learning platform.
The baseline Learning Network experience lasts for 8 months and will start with a virtual meeting in February 2026. Practices selected for the Learning Network will be given the opportunity to continue in the Network after the 8-month preliminary training and learning experience.
