Better Care Playbook Evidence Roundups: Home-Based Primary Care, Hospital at Home for Medicaid Enrollees & Medicaid Health Home Collections
The Better Care Playbook has published an evidence roundup resource collection focusing on home-based primary care, as well as two new evidence roundups on Hospital at Home programs for Medicaid enrollees and Medicaid Health Homes for individuals with behavioral health conditions.
Millions of adults in the U.S. are either homebound or have difficulty leaving their residence. High-quality home-based primary care (HBPC) can help provide person-centered care to underserved populations, including in rural settings, and curb costs associated with unnecessary hospital visits or nursing home admissions.
Health care stakeholders can use this HBPC Playbook Collection to understand the evidence behind the approach and explore practical tools and case studies to learn effective strategies for implementing these programs. Content is organized in three buckets:
- What works in home-based primary care?
- What do these models look like in practice?
- How can my organization implement this approach?
The HBPC collection was recently refreshed with new resources and tools to support implementation.

Hospital at Home for Medicaid Enrollees: This collection explores how states are adapting to Hospital at Home programs for Medicaid enrollees, including dually eligible individuals, with insights from the CMS Acute Hospital Care at Home initiative. The roundup includes early lessons, evidence and policy updates.
Medicaid Health Homes for Individuals with Behavioral Health Conditions: This collection highlights how states are using the Medicaid health home model to better support people with behavioral health needs, with examples of different approaches and evidence on impact across states.
