Transitions in Care 2.0: Family Caregivers and Systems Change
This full-day conference sponsored by the United Hospital Fund will examine in depth how to strengthen transitions in care programs by more effectively including family caregivers. The conference will bring together leaders of major national program models—including Hartford grantees Eric Coleman, MD, MPH, and Mary Naylor, PhD, FAAN, RN—to assess opportunities and challenges to engaging family caregivers.
This full-day conference will examine in depth how to strengthen transitions in care programs by more effectively including family caregivers. The imperative for meaningful change will be addressed by the presentation of the United Hospital Fund’s “Agenda for Change.” The conference will bring together leaders of major national program models—including Eric Coleman, MD, MPH, and Mary Naylor, PhD, FAAN, RN—to assess opportunities and challenges to engaging family caregivers. In addition to a panel of family caregivers identifying what changes they would want, the conference will examine several initiatives that have improved hospital and community care.
