U.S. Department of Veterans Affairs: What Matters Video Series
The Veterans Health Administration (VHA) Geriatric Research Education and Clinical Center has released a video series, "Serving Older Veterans," on What Matters Most.
The series - produced by the VHA Institute for Learning, Education, and Development - features four stories about having "what matters most" conversations with your healthcare team. Talking about your priorities is the first step in creating a health care plan that focuses on what's most important to you.
Each video in the collection shows a different person's journey, exploring what matters most to each person. "What Matters" is one of the 4Ms of the Age-Friendly Health Systems (AFHS) Framework. This concept is consistent with initiatives from Age-Friendly Health Systems, Patient Priorities Care and Whole Health (VHA Office of Patient-Centered Care and Transformation).
