Webinar: Take Charge - Tips for Advance Care Planning
The Family Caregiver Alliance is hosting a webinar about advance care planning on October 30, "Take Charge: Tips for Advance Care Planning."
The webinar will teach participants how to discuss their wishes for care with the most important people in their lives and how to choose a health care proxy. Guidance will be offered on how to document care wishes using Advance Heath Care Directives and the Physician Order for Life Sustaining Treatment (POLST) form.
Mary Matthiesen, community engagement and education director with Mission Hospice & Home Care in San Mateo, CA, will lead the online seminar.
