The Conversation Project, in partnership with Ariadne Labs, has updated the workbook, What Matters to Me, to help people with a serious illness think through and talk about what matters most to them.

The workbook, one of several resources from The Conversation Project, is designed to help people with a serious illness get ready to talk to their health care team (doctor, nurse, social worker, etc.) about what is most important to them — to make sure that they get the care they want.

In this new version feedback has been incorporated from users to include simpler instructions, suggestions for family caregivers, new examples, and more concrete next steps.

The Conversation Project has advice on talking to your health care provider in How To Talk To Your Doctor, and a workbook for those who are not seriously ill called Conversation Starter Kit.
