Continued Development of the Better Care Playbook

Grant Summary

This renewal grant continues the Foundation’s partnership with four other funders to improve outcomes for adults with complex health and social needs by helping health care organizations participating in value-based payment adopt evidence-based models and practices. As one step toward this aim, the five foundations - The Commonwealth Fund, The John A. Hartford Foundation, the Peterson Center on Healthcare, the Robert Wood Johnson Foundation, and The SCAN Foundation - have partnered with the Institute for Healthcare Improvement to create the Better Care Playbook, an online tool for health system leaders that is now populated with approximately 180 resources. The Playbook provides users with the best available knowledge about promising approaches to improve care for people with complex needs and encourages users to test best practices in their own care settings.
