The American College of Physicians Patient Priorities Care Curriculum

The American College of Physicians (ACP), in partnership with the Patient Priorities Care team, now offers free and updated online modules to prepare clinicians and other health care team members to implement Patient Priorities Care in practice.

The three Patient Priorities Care modules on ACP are:

  • Introduction to Patient Priorities Care
  • Helping Your Patients Identify Their Health Priorities
  • Aligning Care and Decision Making with Patients’ Health Priorities.

Decision-making with your patients who have multiple chronic conditions is complicated. The benefits and harms of many interventions are uncertain because of the effect of competing conditions and lack of evidence; conflicting recommendations occur when clinicians focus on different conditions. These patients also vary in the outcomes that matter most to them and what healthcare they’re willing and able to do. Patient Priorities Care addresses these issues by providing an approach to decision-making that involves patients identifying their health priorities and clinicians aligning their decision-making to achieve these health priorities. The resources provided address the What Matters “M”, the core element of the Age-Friendly Health Systems (AFHS) 4Ms Framework: what Matters, Medication, Mentation, and Mobility.
