American Geriatrics Society Co Care Hospital Elder Life Program and Age Friendly Resources

The American Geriatrics Society (AGS) CoCare®: HELP (Hospital Elder Life Program) has released resources to support hospitals and health systems in providing age-friendly care.

AGS CoCare®: HELP (Hospital Elder Life Program) is a comprehensive program designed to prevent delirium and functional decline in hospitalized older patients, giving institutions the tools and resources to support implementation.

AGS CoCare®: HELP & Age-Friendly builds on this by advancing the Age-Friendly Health Systems (AFHS) initiative to ensure all care with older adults is age-friendly care. This means reliably providing the AFHS 4Ms Framework (What Matters, Medication, Mentation, Mobility) every time an older adult and their caregivers interact with the health system. The 4Ms are a framework to guide all care, supporting older adults’ strengths and wellness across cultures, conditions and settings.
