Convergence Collaborative on Social Factors of Health Reports: On Community Hubs and On Nursing Homes

Convergence Collaborative on Social Factors of Heath

The Convergence Center for Policy Resolution (Convergence) has released two resources summarizing discussions from its Collaborative on Social Factors of Health about the potential role of community hubs in coordinating health and social services.

Through a three-meeting series, the Collaborative convened nearly 40 leaders from multiple sectors to brainstorm how community hubs could be leveraged to best serve local populations.

Summary of Brainstorming Meetings on Community Hubs reflects themes and questions from discussions on how hubs can coordinate a range of health, housing and other social services, and examines different models and functions for these organizations.

Could Nursing Homes Become Hubs in Some Communities? highlights a related discussion that explored whether some nursing homes might evolve into community hubs in certain contexts. It shares examples of facilities expanding services or partnering with other organizations to support a broader range of health and social needs in their communities.
