HCCI Blog: The Power of Three - Grant Recipients Continue to Advance Home-Based Primary Care

The Home Centered Care Institute (HCCI) has published a blog, "The Power of Three: Grant Recipients Continue to Advance Home-Based Primary Care."

The blog describes the work of three organizations - the American Academy of Home Care Medicine (AAHCM), the National Home-Based Primary Care Learning Network, and HCCI - which are closely collaborating with the goal of increasing access to high quality, home-based primary care that leads to improved outcomes for both patients and caregivers. The article explains the organizations' respective home-based primary care projects, which include an online directory, a learning network, and education and training, and talks about current progress.

The John A. Hartford Foundation is supporting this work with a three-year grant, "Moving and Scaling Home-Based Primary Care Phase II: Quality, Training and Advocacy," that aims to improve care for the more than two million older adults who are the “invisible homebound” with functional impairments and frailty.
