HCCI Illinois House Call Project Call for Nominations: Cohort 2
The Home Centered Care Institute (HCCI) is offering Illinois practices a period to be considered, or nominate another practice, for applying to the second cohort of the ILHCP that will launch in early 2024. The nomination process is open until January 12.
The project goal is to identify, equip, and motivate a cohort of practices dispersed across Illinois currently providing home-based primary care (HBPC), and those looking to expand or add HBPC as a new service, to participate in a 3-year program aimed at increasing the number of Illinois residents receiving high-quality HBPC. The new goal of the ILHCP is to collectively enroll 6,000 new homebound or home-limited people in their homes from 2022 to 2026.
Cohort 2 will aim to identify regional disparities in access to HBPC, increase patient referrals, foster the growth of high-performing HBPC, support the recruitment on new providers and raise awareness among the Illinois public regarding the value of HBPC. This cohort will also include palliative and hospice care. Each selected practice will designate up to three “Home Centered Care Champions” to lead their work, and HCCI will provide expertise, guidance, and comprehensive resources.
Selected nominees will have the opportunity to participate in a priority application process, before the early 2024 kick-off of the next ILHCP cohort.
