Healthcare Association of New York State: My Health Checklist Patient Education Resource Videos
The Healthcare Association of New York State (HANYS) has released ready-to-use patient education videos to promote use of the Age-Friendly Health Systems tool My Health Checklist.
Created by the Institute for Healthcare Improvement, My Health Checklist is a guide that helps patients and care partners develop a plan of care centered on what matters most. HANYS' patient education videos aim to promote use of the checklist and support more meaningful, person-centered conversations between older adults, caregivers and care teams.
HANYS patient education video resources:
- Waiting room video - intended to be shown in waiting rooms and other public-facing areas to introduce My Health Checklist and encourage patient engagement.
- Social media video - designed to promote awareness of My Health Checklist on social media.
- Implementation guidance - provides practical guidance for deploying the checklist and videos across care settings.
HANYS encourages teams to leverage these free videos and incorporate them into broader patient education and engagement strategies.
