Home Centered Care Institute: Seventeen Medical Practices Selected for Illinois House Call Project
Seventeen medical practices have been selected for the Home Centered Care Institute (HCCI) Illinois House Call Project.
The goal of the project is to identify, equip, and motivate a cohort of practices dispersed across Illinois currently providing home-based primary care (HBPC) — and those looking to expand or add HBPC as a new service — to participate in a 3-year program aimed at increasing the number of Illinois residents receiving high-quality HBPC. These practices will become part of the HCCI Practice Excellence Partner (PEP) Network. HCCI PEP sites are leading house call programs recognized nationwide as premier HBPC providers.
The selected Illinois medical practices, some independent, others representing hospital systems, and three supporting subspecialty groups including dentistry, dermatology, and wound care, will build on their track record of caring for the state’s most vulnerable residents—homebound adults with chronic conditions. Each practice has designated up to three "Champions" to lead their work in the project. HCCI will provide the Champions with ongoing professional education to develop their skills, mentor others in best practices, and raise awareness about the value of HBPC in their communities.
