IHI Diagnostic Excellence: Age-Friendly Health Systems Seed Grant Program Cohort 1 Awards and Cohort 2 Request for Proposals

The Institute for Healthcare Improvement (IHI) has awarded the Diagnostic Excellence – Age-Friendly Health Systems Seed Grant Program to seven health care systems in Cohort 1. IHI is now accepting applications for Cohort 2 of the program.

The grant was awarded to test new and novel ways to improve the care and outcomes of older adults through the reliable implementation of best practices and innovations supporting age-friendly diagnostic practices, aligned with the 4Ms of Age-Friendly care (What Matters, Mentation, Medication and Mobility).

The Cohort 1 selected health systems are Banner Health, Baylor College of Medicine – Harris Health System, Brigham and Women’s Hospital, Native Healthcare Center, MedStar Health, Michael E. DeBakey VA Medical Center and the Veterans Affairs Maryland Healthcare System.

Grantees were selected for their ability to support non-geriatricians in various settings, foster meaningful engagement of older adult patients and families, and improve equity and mitigate disparities, including ageism, race/ethnicity, gender and/or social determinants of health.

IHI is accepting applications for the Cohort 2 of the Seed Grant Program and is hosting an informational webinar on October 17 at 12pm ET. The application deadline is November 8.

This grant builds on the previous Society to Improve Diagnosis in Medicine Diagnostic Quality Improvement Seed Grant Program and is supported by JAHF and the Gordon and Betty Moore Foundation.
