NDCC Resources: GUIDE Model Videos for Clinicians, Caregivers and Families
The National Dementia Care Collaborative (NDCC) has released two new videos about the Centers for Medicare & Medicaid Services' (CMS) Guiding an Improved Dementia Experience (GUIDE) Model, one for people living with dementia and their caregivers and one for primary care providers.
The CMS GUIDE Model is a voluntary nationwide model designed to improve quality of life for people living with dementia, reduce strain on caregivers, and help people remain in their homes and communities through coordinated care and support.
In the video for families and caregivers, Marie Clouqueur, LICSW, of McLean Hospital Geriatric Psychiatry, explains how GUIDE connects families with a care navigator who can help them understand care options, access resources and plan for changing needs.
The video also includes reflections from Donna and Bob, who share their family's experience with dementia care and speak to the value of having guidance, support and someone to turn to throughout the dementia care journey.
In the video for primary care providers, Barak Gaster, MD, of the University of Washington, and Leslie Evertson, DNP, GNP-BC, FAAN, of UCLA, discuss how GUIDE can add another layer of support for primary care teams caring for patients living with dementia and their caregivers, helping families navigate care needs, connect with resources and access assistance beyond the clinical visit.
