Videos on Home-Based Primary Care
Two short videos explaining home care medicine have been released by The American Academy of Home Care Medicine (AAHCM) and by the Home Centered Care Institute (HCCI).
The two videos explore what home-based care is and the interdisciplinary teams that provide home-based medicine. They talk about the needs of older adults and the benefits care at home provides to people who are home-bound. HCCI discusses palliative care at home and how this intersects with home-based primary care, and discusses opportunities that new payment models, particularly value-based payments, offer.
