The Coalition to Transform Advanced Care Blog: Recent Blogs on Advance Care Planning
The Coalition to Transform Advanced Care (C-TAC) has posted some blogs on advanced care planning. They range from an account of a pre-conference intensive session on making advance care planning part of your services portfolio, a personal account of end-of-life by Elder Overton and a review of Federal advance care planning policy.
Advanced Care Planning is one of many integral, foundational services that underpins the value of person-centered care that C-TAC and its partners embrace.
- "C-TAC Presents Advance Care Planning at USAging Conference." The pre-conference intensive session - Making Advance Care Planning Part of Your Services Portfolio - was hosted by C-TAC in partnership with USAging's Aging and Disability Institute at the Answers in Aging Conference. The intensive workshop talked about advance care planning from the ‘big picture’ to the details of ‘how to.’ The blog details the speakers and panelists and includes a link to the session slides. The session concluded with the joint partnership promising ongoing support and a toolkit to help attendees establish their ACP programs.
- "Personal Journey with End of Life: C-TAC’s Elder Angela Overton’s Story." The blog by Elder Overton talks about her husband and emphasizes the value of the advance care planning conversations they had as a family. Elder Overton includes links to resources from the Let’s Talk About Advanced Care Planning Program.
- "C-TAC Policy Advisor Co-Authors Review on Federal Advance Care Planning Policy." This describes a recently published review of federal advance care planning (ACP) policy entitled “Federal Advance Care Planning Policy Primer – Key Aspects, Barriers, and Opportunities,” co-authored by Senior Regulatory Advisor, Marian Grant and three of last year’s Health and Aging Policy Fellows. All fifty states have policies regarding advance directives, and federal policy has had important implications on promoting awareness of ACP and its corresponding legal documents such as advance directives. The paper provides an overview of key federal policy aspects and barriers that affect ACP use and highlights key opportunities to improve federal ACP policy.
